Using APA guidelines, each intern should write a summary of the internship. The basic format is:

| Introduction | 1 page |
| Internship Description | 2-4 pages |
| Conclusion/Evaluation | 1/2 - 1 page |
| Recommendations | 1/2 - 1 page |
This report should be submitted to the chairperson to review, then afterwards to the Department Head.
This report is the student's own responsibility, and is required to complete the internship.
